INFORMATION FOR PHYSICIANS 








Members of the American Academy of General Physicians (AAGP), who are in good-standing, and who are in compliance with all resolutions, policies, codes and opinions of the AAGP, may obtain a free web address and free web page from GeneralistPhysician.com.


The free web address that may be obtained by each of such AAGP Members will be: "www.generalistphysician.com/(physician's name)".  For example, the free web address that could be obtained by an AAGP Member named John James Doe, M.D. would be: "www.generalistphysician.com/johnjamesdoemd".  When any person would type this address into his/her browser, the browser would immediately go to the web page for John James Doe, M.D.  (It is important to remember that all letters in the web address must be in lower case.  If any letters are typed in upper case, then it will not go to the appropriate page.)


The free web page that may be obtained by each of such AAGP Members will contain the following:

(1) The member's name, address, telephone number, fax number, email address and, if such member already has a website, a link to that member's website.  (Each AAGP Member should use only the address, telephone number, fax number and email address he/she would want his/her patients to utilize.  For example, if a member works in a clinic or office, then he/she would want to use the address of the clinic or office.)

(2) A link to a map showing where the member's address is located on the map with the ability for the browser to enter his/her address and obtain detailed instructions on how to drive from his/her address to the member's address.  (This map and directions are provided via MapQuest.)

(3) Periodically changing clinical news.  Periodically, GeneralistPhysician.com will change the clinical news on each member's web page.

(4) A link to a page which contains information for patients.  On this page, there are links to professional societies, certifying boards, and over one hundred forty different patient-advocacy organizations.  (GeneralistPhysician.com is planning to add additional information for patients on this page in the future.)

(5) A statement on the member's page stating that the member is a member of the AAGP.

(6) A link to the Home Page of the AAGP's website.

Also, be sure to try out the link to the map underneath the address for John James Doe, M.D.  After you have viewed the sample web page click on "BACK" on your browser a sufficient number of times to return to this page.

All of such AAGP Members, who have obtained a free web address and free web page from GeneralistPhysician.com, are encouraged to use their web addresses on all of their professional literature, such as letterheads, business cards, brochures, etc.


Any AAGP Member, who is eligible to obtain such a free web address and web page, and who would like to go ahead and obtain his/her free web address and web page now, may do so by completing the following application form and clicking on the "SUBMT" button below.  After the following completed application form is submitted, GeneralistPhysician.com will construct the web page and email to the AAGP Member information about his/her new web address.  (Please allow up to one month for GeneralistPhysician.com to construct the page.)


In completing the following application form, please remember to enter only the address, telephone number, fax number and email address that patients should utilize to contact you.  For example, if you work in a clinic or office, then you should enter the address, telephone number, fax number and email address of the clinic or office. (If you do not currently have an email address for your patients to utilize, then you can obtain a free email address at a number of web sites.  Many AAGP Members have obtained free email addresses at Yahoo.com and Hotmail.com.)
